For asylum seekers
Appointing a trusted person (PDF, 83.64 Kb) With this form asylum seekers can provide the identity of a person other than their lawyer who will assist them during the processing of their application for asylum.
This form must be duly completed, signed and sent by fax (02/205.50.33) or mail to the head office of the CGRS (Boulevard du Roi Albert II, 26 A, 1000 BRUSSELS). It can also be submitted to the CGRS in person at the Helpdesk, Monday to Friday from 8 a.m. to 11.45 a.m. and from 1.15 p.m. to 4 p.m.
Change of chosen place of residence (PDF, 35.79 Kb) With this form asylum seekers can notify the new address where they wish to receive the mail related to their procedure.
This form must be duly completed and signed, and sent only by registered mail, to the head office of the CGRS (Boulevard du Roi Albert II, 26 A, 1000 BRUSSELS) and to the head office of the Immigration Department (World Trade Center, Chaussée d’Anvers, 59 B, 1000 BRUSSELS). It can also be submitted to the CGRS and the Immigration Department in person at the Helpdesk, Monday to Friday from 8 a.m. to 11.45 a.m. and from 1.15 p.m. to 4 p.m.
Waiving assistance from an interpreter (PDF, 91.67 Kb) With this form asylum seekers can waive assistance from an interpreter. The procedure will then take place in Dutch or French, according to the language that was chosen by the representative of the Minister when the application was made.
This form must be duly completed, signed and sent by fax (02/205.50.33) or mail to the head office of the CGRS (Boulevard du Roi Albert II, 26 A, 1000 BRUSSELS). It can also be submitted to the CGRS in person at the Helpdesk, Monday to Friday from 8 a.m. to 11.45 a.m. and from 1.15 p.m. to 4 p.m.
Waiving the application for asylum (PDF, 91.36 Kb) With this form the asylum seeker can end the asylum procedure.

For recognised refugees
Requesting certificates or refugee certificates (PDF, 37.99 Kb) With this form recognised refugees can request a (birth, marriage, etc.) certificate or a refugee certificate.
This form must be duly completed, signed and sent by fax (02/205.52.01) or mail to the head office of the CGRS (Boulevard du Roi Albert II, 26 A, 1000 BRUSSELS). It can also be submitted to the CGRS in person, Monday to Friday from 8 a.m. to 4.30 p.m.
